1. Download the  Abstract Submittal Form ( PDF, 60KB) and the  Abstract Template ( MS Word, 31KB).
2. Use the template and follow the formatting instructions in it to write your abstract.
3. Complete the  Abstract Submittal Form ( PDF, 60KB) using one of the following methods:
   1. Open the form in Adobe Reader, print it and type/ write the required info on the printout, then fax it to us (contact details follow).
   2. Open the form in Adobe Reader, fill the form fields in Adobe Reader (no pen/type machine needed!), print the form and send it to us by fax. You will not be able to save a copy of the filled document and must keep a copy of the printout for your records.
   3. If you have the Adobe Acrobat editing software, you can do as instructed in option 3.b.; you will be able to save the completed form for your records on your computer.
4. Please fax, mail, or e-mail the Abstract Submittal form along with your abstract to Linda Legerski by September 27, 2010.
